Xmo Strata has implemented a number of initiatives to minimise the risk of blade cuts, having identified in 2008 that they equated to 66% of incidents resulting in harm.

LOTO Testing
This highlights the need to always test any equipment you isolate to ensure it is actually dead before working on it. Just because you have isolated and locked something off, it doesn't necessarily mean the equipment is now safe to work on.

Grinders
All tools must be used in accordance with the manufacturer's instructions, and no components / guards may be removed or modifications made. If any tools have been damaged or safety shields / guards are missing, contact your supervisor for a replacement.

Insect Stings

Sun Induced Skin Cancer
At this time of the year, we all enjoy the sun, but need to be acutely aware of the effect it can have on our skin. Over exposure to the sun is the biggest cause of skin cancer, one of the most common cancers in the UK; there are two main types, malignant melanoma of the skin and non-melanoma skin cancer (NMSC) and they are extremely common.

Peer On Peer Audits
At our safety awareness day in December, we announced the introduction of peer on peer audits where site operatives audit each other using the F1.3.1 audit form that was previously only used by the management team. The objective of this is to encourage more time “talking” about safety issues, as opposed to ticking boxes.

Recent observations on site have identified that many contractors, including our employees, arrive on a live site, put on their PPE and walk across the forecourt to sign-in.

Live Site Works
Most of the sites we work on are 'live' - they are open for business and have members of the public driving on and off site. Frequently the public do unexpected or illogical things, including attempting to enter barriered off work areas.

Hand Injuries In 2009
As you are all aware, Xmo Strata rules require gloves to be worn at all times whilst on site and all JSAs identify the minimum rating of the gloves to be worn for each task. If you have concerns or disagree with any rating on the JSAs or require a new hand protection checklist card please let your supervisor know.

LOTO Incidents
In March, Xmo teams were undertaking re-imaging work on a filling station that was being developed. Prior to the team attending, and as part of the overall site scope, a nominated electrical contractor had attended site to isolate the electrical feeds.

Xmo First Quarter Safety Review
I am pleased to confirm that we've had our best ever start to a New Year by identifying 67 hazard spots/potential incidents/near misses and have suffered no accidents or incidents resulting in anyone being harmed. At the time of identification we were able to mitigate 34 potential hazards totally, and reduce the risk on a further 12 (a residual risk analysis is included later).

Chesterfield UK Lessons Learnt
Whilst Xmo Strata were not involved in any way, the Chesterfield incident is a reminder that barriers must always be used to protect the work area, and any hazards must be identified and addressed - if somebody on this site had spotted this hazard and corrected it they would have prevented an injury.
