Year End Safety Awareness

As we near the end of yet another eventful year, we must remain conscious of the very high levels of activity we are likely to encounter across the region. We must exercise greater caution during the cold and dark days ahead which bring testing conditions to be managed safely.

Cut Finger

An engineer was collapsing a cardboard box and needed to cut the tape holding the base together. The engineer held the box with his left hand and cut the tape downwards. The box was held at approx 45deg from the body so the safety knife was not being pulled towards the body; however, the engineer's hand was in the arc the knife was travelling through.

Using Defective Equipment

This shows how dangers can arise through the use of defective equipment. Always inspect all equipment before use, especially electrical equipment. If you find any faults, do not use the equipment and report it to your supervisor. Do not try to compensate for or repair the defect; as can be seen in this report they noticed the drill was damaged, and thought they could compensate for this, but ended up being injured.

Getting The Job Done

The enclosed alert shows how dangers can arise on-site by people not thinking though their actions and just “getting the job done”. Any deviation from the SMS/JSA/JHA must be considered and documented on your clearance certificate. This is not just true at the start of the job when you arrive on-site - if anything changes during the day, or you find something is not as you expected, carry out a LMRA (Last Minute Risk Assessment) and document the change(s) on your clearance certificate.

Site Robbery

A robbery took place on a construction site. Two men in a white transit van tricked the site security guard into letting them on site at 20:00 hrs. While one acted as look out, the other moved equipment including drills and a compactor machine from the storage cabin into their van.

Pre-Use PPE Checks

Although we do not use this exact anchorage connector, similar crimped connectors are very common, and it would be easy for a similar incident to happen where a new component is just assumed to be in good condition.

On-Site Risk Assessment

In a recent Health, Safety & Environment Bulletin (No 114), we discussed the issues found when we carried out a random audit of clearance certificates. The issues included very generic and woolly hazards being identified with insufficient detail as to the control measure adopted. A common example was given where the hazard was identified as "Traffic" and the control measure was "PPE" - this is obviously insufficient as wearing ear defenders, as an example (PPE), will have no effect on the traffic hazard (and could actually make it more dangerous).

Log Out Tag Out

As part of our ongoing Health and Safety incident reviews, we have noted that the number of electrical related near-misses had more than doubled, increasing from 6 near misses in 2007 to 13 in 2008. Although most of these are reports of near-misses unrelated to Xmo work, they highlight the significant risk that electricity poses to all contractors and sometimes the general public on site; electrical incidents represent 14% of all the near-misses reported to us.
