This highlights the need to always use the right tool for the job and to put control measures in place to manage any hazards that are found. In this case the worker carried out the initial part of a LMRA (Last Minute Risk Assessment) - he identified the shelf as a hazard, but he failed to put in place any form of control measure, or alter his work plan, to take into account this hazard. Risk assessments are pointless unless you control the hazards that you identify.
