Near miss reporting

Xmo Strata is pleased to report that it is now staring to make steady progress in recognizing and reporting near miss occurrences on site.

At this point in time, the company has identified and reported 91% more Near Misses than it did in the whole of 2006. Please note that some near misses can be categorized with more than one immediate cause or substandard condition, hence this not being reflected in the graph shown below.

The key on the bottom right-hand corner of the page identifies the immediate cause of the incident, whilst the hatching around the pie chart indicates whether it relates to an employee, subcontractor or third-party (over which Xmo Strata has no authority i.e. a member of the public or contractor not related to its works).

One trend that is noticeable from the data on the pie charts extracted from our reporting data is that we appear to be noticing near misses relating to third parties and sub- contractors more than those relating to Xmo Strata or its colleagues. Anyone who would like further explanation of how to understand the data is welcome to contact Xmo Strata to discuss this.

Near miss reporting is encouraged with the objective of establishing a ‘no blame’ culture. This allows the company to recognise possible areas of error, and put in place strategies of how to better the identified areas. Staff are encouraged to be self-critical and report any instances in which the company could implement safer conditions to prevent loss to either life, wellbeing or property.

The specific objective of the management team is to achieve an Incident and Injury Free working environment on all Xmo Strata sites, and to ensure that all colleagues, sub contractors, site staff and customers all go home safely to their families each day.
